We offer a transition training program for parents and caregivers to develop or enhance the skills required to care for their loved one at home. Our transition program is designed specifically to meet the individual needs of each resident and family on their journey back to their home.

Our primary goal is for each resident to achieve a greater quality of life and provide maximum growth and development through our therapy services.

Through 24/7 support from our highly trained staff of nurses and aides, we are able to provide excellent care for residents including:

• DDD and AHCCCS qualified members

• Trach and ventilator dependent members

• Members requiring tube feedings, intravenous therapy, peripherally inserted central catheter (PICC access) or wound care

• Patients who require continuous medical assistance
